What to Bring/Submit Electronically [Distance Learning]
*During Distance Learning, please email all completed documents to email@example.com
- Completed Fundraiser/Activity Application. Please have all applications thoroughly completed with ALL details and signatures. Approvals will occur at the meeting.
- Club/Program Meeting Minutes documenting student discussion and approval of the following items:
- Club/Program Activities (not requiring fundraising, ex. tabling event, spirit day)
- Club/Program Expenditures (purchases, reimbursement needs, etc.)
- Club/Program Fundraising
Please use one of the meeting templates provided by the ASB.
- Club/Program Meeting Sign-In Sheet showing student attendance and participation at the meeting.
- Voluntary Participation Form. Please have a completed form for each fundraiser. Only one copy needs to be submitted to be kept on file for auditing purposes. Individual forms completed and signed by students and parents should be kept on file with the club/program advisor
***All four documents should be submitted [electronically during Distance Learning to firstname.lastname@example.org] as one complete application. Please keep copies for your records.
